You can call a benefit advisor at the Washington office of the Department of Labor’s Pension and Welfare Benefits Administration (PWBA) at 202-219-8776 or in the PWBA regional office nearest to you.

You can visit the Centers for Medicare & Medicaid Service (CMS) of the Department of Health and Human Services or contact your State Insurance Commissioner’s office.
